Your Sun Health team began 2014 in full force – mobilizing several signature projects. It’s exciting to see so many activities coming together that will make a positive impact for many in communities we serve.

The Sun Health operating theme of “population health” is policy in action. It is the foundation for all we do to support you, our independent living residents, and area “friends and neighbors,” who trust us to enhance their well-being and our partner hospitals.

Each year, your Board of Directors participates in a “retreat” day for education and reflection on the accomplishments of the past year and goal setting for the year ahead. The focus this year will be on an interactive discussion on health care policy, clinical excellence and dynamic innovations required to support the next generation of health care delivery. SHINE Delivers Toys to 100+ Children from Craniofacial Foundation of Arizona

More than 100 kids with craniofacial deformities were showered with gifts at the Craniofacial Foundation of Arizona’s (CFA) annual holiday party, held Dec. 15 at St. Joseph’s Hospital and Medical Center in Phoenix. The gifts were donated to CFA by Sun Health employees and residents through the organization’s SHINE (Sun Health Involvement Never Ends) committee, which held a November toy drive to gather the presents.

Sun Health employees wrapped almost 100 of the gifts for the kids and donated dozens of unwrapped toys to CFA. Lori Takeuchi, program coordinator for CFA, was deeply touched by the generosity.  Continue reading

Crowds Bunch for Holiday Boutique & Home Tour

More than 400 guests toured six showpiece homes and shopped for holiday treasures at Clark home 4 - Copythe 2013 Holiday Boutique & Home Tour, held Dec. 7, at The Colonnade in Surprise. The events were sponsored by Sun Health Foundation to benefit cardiac care at Banner Boswell and Banner Del E. Webb medical centers.

The tour of the homes was a “tour de force” featuring everything from modern to traditional architecture, priceless works of art and furnishings from around the world. Guest also enjoyed appetizers and refreshments at each home. The two-day gift boutique offered shoppers a wide assortment of holiday-themed gifts.

How do we make it happen? Here is a snippet of what CEO Ronald Guziak had to say when recently interviewed on ThinkHippo.

With your Care Transitions program, you had some great statistics – notably, the 85 percent reduction in readmissions. As this model continues to grow, what sort of changes do you expect in staffing? People are the most important thing. As the Care Transitions program grows, our focus is to hire the right people to do the work the way we want it done. We’ll continue to use RNs or LPNs to initiate that first stage of the program once the patient is discharged. We’ll remain committed to providing home visits, not just telephone visits – those are the things that differentiate the program. As we begin to serve different generations, we’ll add more technology to the services we provide. We also look at ways to offer support for 90 days and beyond.

La Loma Care Center sporting a new look

Since opening its doors in 2005, La Loma Care Center has provided its residents and the surrounding community with high quality rehabilitation and long-term care services. In 2013, the center launched a major re-positioning project to upgrade its campus and prepare for future skilled nursing and assisted living needs based on projections.

The project included remodeling existing space into a 6,400 square-foot, state-of-the-art rehabilitation gym, equipped to serve inpatients–and for the first time, outpatients. The center also opened a new 27-bed short-stay skilled nursing unit, featuring home-like touches and patient-centered nursing care. A newly constructed Community Education Room recently opened and work is nearing completion on 10 new assisted living apartments, with residents expected to move in in January or early February 2014. Rebecca Bair Joins Foundation as Senior Director of Development
