Pathways to Population Health

philanthropyPhilanthropy – Through our partnership with Banner Health, Sun Health remains committed to providing charitable giving and volunteer resources for Banner Boswell Medical Center, Banner Del E. Webb Medical Center and Banner Sun Health Research Institute. Following the formation of our partnership in 2008, Sun Health has contributed more than $27 million to health and human service programs for Banner Health and a variety of other community agencies serving the Northwest Valley.

 

communityprogramsCommunity Programs – An advocate for healthy living, Sun Health offers free educational seminars and has developed two programs designed to help individuals better manage their chronic disease and medication regimens: Sun Health Medication Management and Sun Health Care Transitions. Initial results show patients are avoiding readmission to the hospital, better managing their chronic diseases at home and in some cases, even saving money on their prescription medications.

 

seniorlivingSenior Living – More than 700 residents call Sun Health Senior Living home at The Colonnade in Sun City Grand/Surprise, Grandview Terrace in Sun City West and La Loma Village in Litchfield Park. Consistently boasting “excellent” ratings from the Arizona Department of Health Services and the Centers for Medicare and Medicaid, our communities also offer Masterpiece Living. The nationally recognized program and research-based way of life focuses on four specific areas proven to contribute to healthy aging: physical, social, intellectual and spiritual.

 

superiorhealthcareSuperior Health Care – Sun Health has targeted its health care investments toward key hospital and research priorities serving our Banner Health partners: Cardiology, Neuroscience, Orthopedics and Research. Most recently, we celebrated the opening of the new Cath Lab and behavior health unit at Banner Del E. Webb Medical Center. We also continue our work on cardiac stem cell research at Banner Sun Health Research Institute and developing surgical suites at Banner Boswell.
