Sun Health Care Transitions Strengthens Growing Team with Addition of New LPN

Sun Health HIres RN Melissa WallerSun Health’s nationally recognized Care Transitions program continues to grow to meet the needs of West Valley patients. The nonprofit program proudly announces the hiring of Licensed Practical Nurse Melissa A. Waller of Peoria.

Waller brings 21 years of well-rounded nursing experience to Care Transitions, a program designed to help patients navigate through the difficult and sometimes daunting experience of living with a chronic illness.

Most recently, Waller served for more than six years as a clinical office coordinator and field LPN for Hospice of the Valley. Waller has also worked as a charge nurse and co-unit coordinator for a nursing and rehab care center in Sun City West.

Prior to moving to Arizona, Waller worked for a variety of medical practices in Virginia. She earned a practical nursing diploma from Chesapeake Center for Science and Technology in 1993, and she also holds an associate’s degree in paralegal studies.

Waller enters a program that in its second year has served more than 2,200 patients in our community and has helped reduce readmissions by 67-percent.  Care Transitions’ success has been recognized nationally, earning consecutive “Outstanding Leadership in Population Health Award” from the Care Continuum Alliance (CCA). Representing more than 200 health companies, organizations and individual providers committed to improving the health of populations, the CCA focuses on raising care quality, improving health outcomes and reducing costs for individuals affected by chronic disease.
