In late November, Sun Health’s Care Transitions program was prominently featured in Jordan Rau’s The New York Times article “New Efforts to Close Hospitals’ Revolving Doors.” The story included a photo of Sue Koner, a transition care manager for Sun Health, checking patient Ted Cohn’s blood pressure during a home visit.
